A New Lease on Life for the “Blue Pill”: How Regular Viagra Helps Stop the Spread of Metastases

The well-known and popular blue pill may turn out to be much more potent than previously thought. Recent scientific studies suggest that sildenafil, the active ingredient in the erectile dysfunction drug “Viagra,” may also have anti-cancer effects.

Israeli experts conducted a series of tests on sildenafil using malignant cells in a laboratory setting and found that the drug prevents the further spread of cancer by blocking the cells’ ability to absorb cholesterol. The authors of the study are convinced that this discovery will pave the way for a new approach to combating metastatic forms of cancer in the future.

The project’s lead author, Ayelet Erez, a researcher of oncological processes at the Weizmann Institute of Science, noted in her official statement to the university that the scientists had succeeded in identifying a previously unknown biological mechanism linking a well-studied signaling molecule to the process of cholesterol regulation within cells. She emphasized that targeting this pathway can prevent cancer cells from forming new metastases.

By their very nature, all cancer cells differ significantly from healthy tissues in many ways. In particular, they often rely on less common chemical reactions to fuel their continuous growth and spread to other parts of the body – a process known as metastasis. The researchers’ primary focus was on determining how tumor cells use cholesterol to sustain uncontrolled growth, as well as whether there is a safe way to halt this process.

The primary effect of sildenafil on the human body is the inhibition of the enzyme phosphodiesterase type 5 (PDE-5), which stops the breakdown of another important molecule – cGMP. Increased cGMP concentrations in the bloodstream promote prolonged dilation of certain blood vessels, particularly in the corpus cavernosum of the penis, which is what makes it an effective treatment for erectile dysfunction. Researchers have found that cGMP also has the ability to bind to a protein responsible for transporting cholesterol within cellular structures; consequently, an increase in cGMP levels reduces cholesterol transport. As it turns out, cancer cells are particularly vulnerable to this type of intervention.

During the experiments, the scientists analyzed in detail the effect of sildenafil on cancer cells from mice and humans in a laboratory setting. It was found that the drug slows down the movement of cholesterol within cancer cells, causing them to lose their ability to metastasize effectively. Subsequently, the researchers examined the medical records of 5 million patients treated by Clalit Health Services – Israel’s largest public healthcare organization. The data showed that cancer patients who were also taking sildenafil had higher survival rates compared to similar patients who did not take the drug.

However, these results alone do not yet constitute definitive proof that sildenafil and similar medications are guaranteed to destroy tumors. Further trials will be needed to definitively confirm these findings and assess whether this effect has real clinical value. Many drugs show promising results against cancer cells in petri dishes but ultimately fail to provide any real life extension for actual cancer patients.

Despite this, the study authors are convinced that they have discovered a fundamentally new method for weakening tumors due to their increased dependence on the cholesterol transport mechanism – a pathway that other drugs besides sildenafil can also exploit. For example, when the researchers combined sildenafil with statins (a class of cholesterol-lowering drugs), this further limited the spread of metastases. Similarly, an analysis of medical records showed that cancer patients who took both types of medication simultaneously had the lowest likelihood of dying from cancer.

In their concluding remarks, the researchers noted that increasing cGMP levels by inhibiting PDE-5a should be considered a potential strategy for limiting metastasis, which provides a clear mechanistic explanation for sildenafil’s positive effect.

If sildenafil does indeed find a new use as a treatment for cancer over time, this will be far from the first example of its unexpected application. Previous scientific studies have already suggested that PDE-5 inhibitors may hypothetically also help in the treatment of dementia.
